ABSTRACT Objective: To describe how local research ethics committees (LRECs) consider and apply research ethics in the evaluation of biomedical research proposals. Design: A qualitative study was conducted using purposeful sampling, focus groups and a grounded theory approach to generate data and to analyse the work of the LRECs. Setting and participants: 11 LRECs of the Mexican Institute of Social Security (IMSS). Results: LRECs considered ethics to be implicit in all types of research, but that ethics reviews were only necessary (...) for projects that included the direct participation of human beings. The LRECs appeared to understand the importance of consent, as in the completion of a consent form, but did not emphasise the importance of the process of acquiring ‘informed’ consent. The committees considered their main roles or functions to be: (a) to improve the methodological quality of research and to verify – if applicable – the ethical aspects; (b) to encourage personnel to undergo research training; (c) to follow‐up research to oversee the adherence to norms and compliance with a specified research timetable. Conclusions: This study provides a valuable insight into how these LRECs understand the ethical review process. The emphasis of the committees was on rules, regulations, improving research methodology and research training, rather than a focus on efforts to protect the rights and well being of research subjects. The results encourage further normative and descriptive lines of investigation concerning education and the development of LRECs. (shrink)

The objective of this study was to describe the cultural domain of ethical behaviours in clinical practice as defined by health care providers in Mexico. Structured interviews were carried out with 500 health professionals employed at the Mexican Institute of Social Security in Mexico City. The Smith Salience Index was used to evaluate the relevance of concepts gathered from the free listings of the interviewees. Cluster analysis and factor analysis facilitated construction of the conceptual categories, which the authors refer to (...) as `dimensions of ethical practice'. Six dimensions emerged from the analysis to define the qualities that comprise ethical clinical practice for Mexican health care providers: overall quality of clinical performance; working conditions that favour quality of care; use of ethical considerations as prerequisites for any health care intervention; values favouring teamwork in the health professional—patient relationship; patient satisfaction scores; and communication between health care providers and patients. The findings suggest that improved working conditions and management practices that promote the values identified by the study's participants would help to improve quality of care. (shrink)

Background The world literature shows that empirical research regarding the process of decision-making when cancer in adolescents is no longer curable has been conducted in High-income, English speaking countries. The objective of the current study was to explore in-depth and to explain the decision-making process from the perspective of Mexican oncologists, parents, and affected adolescents and to identify the ethical principles that guide such decision-making. Methods Purposive, qualitative design based on individual, fact-to-face, semi-structured, in-depth interviews. The participants were thirteen paediatric (...) oncologists, 13 parents or primary carers, and six adolescents with incurable cancer. The participants were recruited from the paediatric oncology services of three national tertiary-care medical centres in Mexico City. Results The oncologists stated that they broach the subject of palliative management when they have determined that curative treatment has failed. Respect for autonomy was understood as the assent of the parent/adolescent to what the oncologist determined to be in the best interest of the adolescent. The oncologists thought that the adolescent should be involved in the decision-making. They also identified the ability to count on a palliative care clinic or service as an urgent need. For the parents, it was essential that the oncologist be truly interested in their adolescent child. The parents did not consider it necessary to inform the child about impending death. The adolescents stated that the honesty of their oncologists was important; however, several of them opted for a passive role in the decision-making process. Conclusion The findings of this study evidence that to achieve good medical practice in low-middle income countries, like Mexico, it is urgent to begin effective implementation of palliative care, together with appropriate training and continuing education in the ethics of clinical practice. In Mexico, diabetes mellitus is the main cause of end − stage kidney disease, and some patients may be transplant candidates. Organ supply is limited because of cultural issues. And, there is a lack of standardized clinical guidelines regarding organ donation. These issues highlight the tension surrounding the fact that living donors are being selected despite being prediabetic. This article presents, examines and discusses using the principles of non-maleficience, autonomy, justice and the constitutionally guaranteed right to health, the ethical considerations (...) that arise from considering a prediabetic person as a potential kidney donor. BackgroundUmbilical cord banks are a central component, as umbilical cord tissue providers, in both medical treatment and scientific research with stem cells. But, whereas the creation of umbilical cord banks is seen as successful practice, it is perceived as a risky style of play by others. This article examines and discusses the ethical, medical and legal considerations that arise from the operation of umbilical cord banks in Mexico.DiscussionA number of experts have stated that the use of umbilical cord goes beyond (...) the mere utilization of human tissues for the purpose of treatment. This tissue is also used in research studies: genetic studies, studies to evaluate the effectiveness of new antibiotics, studies to identify new proteins, etc. Meanwhile, others claim that the law and other norms for the functioning of cord banks are not consistent and are poorly defined. Some of these critics point out that the confidentiality of donor information is handled differently in different places. The fact that private cord banks offer their services as "biological insurance" in order to obtain informed consent by promising the parents that the tissue that will be stored insures the health of their child in the future raises the issue of whether the consent is freely given or given under coercion. Another consideration that must be made in relation to privately owned cord banks has to do with the ownership of the stored umbilical cord.SummaryConflicts between moral principles and economic interests (non-moral principles) cause dilemmas in the clinical practice of umbilical cord blood storage and use especially in privately owned banks. This article presents a reflection and some of the guidelines that must be followed by umbilical cord banks in order to deal with these conflicts. This reflection is based on the fundamental notions of ethics and public health and seeks to be a contribution towards the improvement of umbilical cord banks' performance. (shrink)

Las teorías que se han centrado en la represión del incesto (Freud, Levi-Strauss) o en la existencia de un contrato social (Hobbes, Rousseau) han generado mucha atención en, respectivamente, el estudio de la familia y el Estado, y muy poca en el de las pequeñas asociaciones igualitarias de varones jóvenes. En el presente ensayo, adheriremos a las teorías biológico-antropológicas de Boehm (1999; 2000a; 2009; 2012), Sterelny (2011; 2016) y Marean (2016) acerca de la evolución de la cooperación. Dichos enfoques proponen (...) a las sanciones sociales y a la resolución de conflictos como mecanismos que condujeron, en el Paleolítico superior, a las primeras formas de organización social igualitaria realizadas por las bandas de cazadores-recolectores. Sostendremos que tales estructuras de organización social siguen presentes allí donde bandas y pandillas operan con igual o mayor importancia que otras instituciones (la familia, los cacicazgos, el Estado-Nación). Con ayuda de la extensa literatura sobre modelos ecológicos de violencia y territorialidad, mostraremos que la existencia de bandas y pandillas (desde el Pleistoceno, pasando por la Era Vikinga, hasta la crisis de violencia en Centroamérica a principios del siglo xxi) se explica en gran medida por la competencia por los recursos y es una forma paradójica de organización cooperativa. Meeting 5G high bandwidth rates, ultra-low latencies, and high reliabilities requires of network infrastructures that automatically increase/decrease the resources based on their customers’ demand. An autonomous and dynamic management of a 5G network infrastructure represents a challenge, as any solution must account for the radio access network, data plane traffic, wavelength allocation, network slicing, and network functions’ orchestration. Furthermore, federation among administrative domains (ADs) must be considered in the network management. Given the increased dynamicity of 5G networks, artificial intelligence/machine learning (...) (AI/ML) solutions are strong candidates able to learn, and take quick provisioning decisions upon fast changes in network conditions. Therefore, this chapter presents an analysis of the state-of-the-art solutions for 5G networks’ management, where AI/ML solutions are discussed and compared with traditional methods. Additionally, the chapter provides a technology overview of both standards, and existing solutions regarding the 5G network management, and directions toward the integration of AI/ML in 5G networks. In the predominant pedagogical frameworks of our days, rationalist, voluntarist or sentimentalist reductionisms are seen, from which it is about educating people regardless of their Origin and the singular and unrepeatable originality with which they have been given to existence. Faced with this anthropocentric confinement, Sensitive Education arises so that every person, regardless of their culture and creed, remains sensitive to their Origin and captures their own originality, which in the end is what they must accept and try to manifest (...) with education. When educational systems are not sensitive to the original, they develop falsehood. With the aim of consolidating the validity of the pedagogical-anthropological foundation of Sensitive Education, this study seeks to show how Edith Stein’s argument explains Sensitive Education, although Stein, after her conversion, uses explicitly Christian terminology. Stein’s direct terms serve to describe Education Sensitive to the Holy Spirit, but for those who do not start from a Christian worldview, Education Sensitive to Origin can be a more non-denominational way of approaching the Original. To achieve this objective, an analytical-synthetic methodology has been followed in which essential features of Sensitive Education are proposed and in a hermeneutic manner, arguments are extracted from the writings of Edith Stein that offer support for these essential features. In conclusion, it is sufficiently shown that Edith Stein’s thought provides a pedagogical foundation to Sensitive Education. (shrink)

La Educación Sensible es ayuda para que el “yo” habite en su “hogar interior” y crezca hacia su “apoteosis original” en el “nosotros”, donde se hace cocreador de belleza con libertad, sabiduría y amor. La educación sensible es pedagogía no invasiva pero radicalmente exigente para que la persona acepte desplegar su versión original y vivir con gozo en un “nosotros-maduro”. No es una pedagogía que protege a los sensibles; es una educación que atiende a todas las personas, independientemente de su (...) grado de sensibilidad: alta, media o baja (como intervalos comunes que se manejan en las ciencias de la sensibilidad). La sensibilidad personal es capaz de captar la propia originalidad y disponerse para interpretarla en los dos sentidos: comprenderla y actuarla. La sensibilidad humana necesita la educación sensible para que sea punto de partida de un desarrollo personal auténtico y pleno de la propia originalidad que es alterada y altera la de los “yoes” con quien habita la existencia formando “nosotros”, que a su vez, se impactan con interdependencia. Además de al “yo”, la educación sensible se orienta al “nosotros”, que será tanto más “sensible” en la medida que altera la intimidad de la persona con “amor-maduro” que es fuente de gozo, a pesar de los pesares. Sin educación sensible la vida daña y cuanta mayor es la sensibilidad de la persona, mayor es el daño, pero también es mayor la capacidad para detectarlo cicatrizarlo y ayudar a otros con educación sensible. Así, la educación sensible se manifiesta de tres maneras: como acción sanadora de la persona, destrucción creadora para evolucionar en el tiempo, como la crisálida se transforma en mariposa, y acción desarrollante del propio potencial con originalidad. The challenges of adapting to this new world require universities to establish new actions that strengthen the preparation of their students. The objective of the research is to identify the degree of burnout that students of the Bachelor of Public Accounting (BPA) and Bachelor of Economics and Finance (BEF) programs of the Technological Institute of Sonora are experiencing. The research methodology is quantitative, non-experimental, with a cross-sectional design and descriptive scope. To collect the information, the Burnout Inventory-Student Survey (MBISS) was (...) used, developed by Schaufeli, Martínez, Salanova, GonzálezRomá and Bakker in 2002 in its Colombian adaptation and validation developed by Hederich and Caballero in 2016. The contribution is to determine the situation of the students of these academic programs in three major aspects: Exhaustion, Cynicism and Academic Efficiency, in such a way that interventions are established that support the academic transition of the student. Résumé L’article analyse la série photographique Danse macabre avec jeune fille de Birgit Jürgenssen selon deux axes : le concept de masque et l’enchevêtrement de deux thèmes iconographiques. En s’inscrivant dans un espace archaïsant, où apparaissent des éléments burlesques, les photographies de Jürgenssen des années 1979 et 1980 sont, comme chez Bakhtine, un éclat de rire contre la culture dominante.

Iris Murdoch on philosophy and literature: morality and the limits of text The paper discusses Murdoch’s view on philosophy and literature. It poses two questions. The first question is how Murdoch distinguishes between these two disciplines. Murdoch defends a separation of the two, whereas in fact she crosses borders. Moreover, Murdoch holds that both philosophy and literature are seeking truth. Therefore, I argue, for Murdoch, philosophy and literature are similar from a moral philosophical standpoint. The second question is whether literary (...) and philosophical reflections reach their limits when it comes to describing ultimate moral matters? Here, Murdoch’s concept of textuality is central. I explain Murdoch’s philosophical concept of text by analysing her thoughts on the ban on writing as shown in the novel The Message to the Planet and in philosophical essays, in particular The Fire and the Sun: Why Plato Banished the Artists. Next, referring to Murdoch’s objections to Derrida’s theory of text, I argue that Murdoch’s concept of text is not strong enough to protect her view from moralism. I suggest that perhaps the moralism can be overcome if further analysis of Murdoch’s work would disclose the role of an alternative, expressionist concept of text. (shrink)

The earliest ancient Greek text to narrate the resolution of a large-scale conflict by judicial means is Aeschylus's tragedy Eumenides, first performed in Athens in 458 BC. After explaining the historical context in which the play was performed—a context of acute civic discord and the imminent danger of an escalation of reciprocal revenge killings by the lower-class faction in Athens—this article offers a new reading of the play and asks if it can help us think about the challenges inherent in (...) conflict resolution today. The prosecutors are the Erinyes, the archaic supernatural agents of murder victims; their responsibility, in predemocratic Greece, before the invention of law courts, was to punish murderers. The defendant is Orestes, who has killed his mother but argues that, since she had killed his father, he was acting justly. The judges consist of eleven Athenian citizen jurors plus the presiding god, Athena, whose twelfth vote carries slightly more weight than any of the others. The article concludes that some aspects of the procedure are exemplary, especially Athena's insistence on respecting each side's right to be heard, her sensitivity toward the grievances felt by the defeated Erinyes, and the compensation they are offered. On the other hand, the tragedy clearly shows how difficult it is for a fair legal judgment to be made without a view to larger issues of national expedience, security, and inherent power structures, especially that of patriarchy. John Dewey's ideas on politics derive from his epistemology of inquiry as practical problem-solving. Dewey's philosophy is important for democratic theory because it emphasizes deliberation through questioning. However, Dewey's philosophy shares with positivism the same conception of answering as exclusively the dissolution of questions. While Dewey's ideas are distinct from positivism in important respects, he rejects a constitutive role for questioning by constructing knowledge as problem-solving via experience. The problem-solving ideal lends itself to a scientific conception of politics. Applying Michel (...) Meyer's philosophy of questioning, problematology, to Dewey's logic, reveals that Dewey's theory of inquiry is itself the product of an inquiry, confirming that questioning plays a constitutive role in knowledge. This suggests a problematological reconstruction of knowledge which extends Dewey's ideas by affirming an enhanced status for questioning. Grounding knowledge in the principle of questioning provides the basis for extending the rhetorical analysis of politics. (shrink)

Representationalist theories of phenomenal consciousness have problems in accounting for pain, for at least two reasons. First of all, the negative affective phenomenology of pain (its painfulness) does not seem to be representational at all. Secondly, pain experiences are not transparent to introspection in the way perceptions are. This is reflected, e.g. in the fact that we do not acknowledge pain hallucinations. In this paper, I defend that representationalism has the potential to overcome these objections. Defenders of representationalism have tried (...) to analyse every kind of phenomenal character in terms of indicative contents. But there is another possibility: Affective phenomenology, in fact, depends on imperative representational content. This provides a satisfactory solution to the aforementioned difficulties. (shrink)

The aim of this paper is to explore the connections between semiotics and biology by examining the behaviors of both humans and non-cognizant agents, using Bayesian and heuristic inference. The argument is that higher-level organisms use complex predictive reasoning to deal with uncertainty, while non-cognizant species, such as SARS-CoV-2, rely on an economy-driven heuristic to enter host cells. From this viewpoint, the current pandemic is characterized as a clash of representations resulting from the anthropocentric construction of the self, which neglects (...) the complexities of the natural world. The concept of autonomy is central to many debates in aesthetics. However, exactly what it means to be autonomous in our aesthetic engagements is somewhat unclear in the philosophical literature. The normative significance of autonomy is also unclear and hotly debated. In this essay, I propose a method for clarifying this elusive concept by distinguishing three distinct senses or varieties of aesthetic autonomy: experiential autonomy, competence-based autonomy, and personal autonomy. On this taxonomy autonomy is a context-sensitive concept and autonomy applies (...) to several different moments or stages of aesthetic appreciation. Throughout this critical discussion, important issues concerning the nature of aesthetic appreciation are also explored. (shrink)
